One (1) ADA Federal Law Information Card (this card is only included for Service Dog Style options)

One (1) set of 2 Customized ID Badges (you will need to provide your dog's photo)

You can also add one of our engraved tags for only $3.99 if you wish to save even more.

For your Air-Tec Jr. Tiny Service Dog Mesh Vest, choose your color, then choose your patch, then choose your size. Please measure carefully.

Very Important Note: You will need to get us a photo of your dog for your ID badges. You can either email us the photo or mail it to our physical location if that is easier. Whichever way you choose to get your service dog photo to us, you will need to let us know in the check box.

A note about sizing - please make sure to measure your dog's girth, TWICE, for an accurate measurement. To do this, use a Soft Fabric measuring tape and measure your dog around the chest just behind the front legs. Make sure your dog is standing up while you are doing this. You can also circle your dog's chest with a leash and then measure the leash where you mark it.

The ADA defines a Service Animal any guide dog, signal dog, or other animal individually trained to provide assistance to an individual with a disability. If they meet this definition, animals are considered service animals under the ADA regardless of whether they have been licensed or certified by a state or local government.
